STH NHS Trust Stroke Clinic Evaluation

The National Stroke Service Model (2021) stipulates that people with stroke are reviewed at 6-weeks post discharge from acute stroke care. Prior to April 2023 the service offered within Sheffield comprised a medical review at the Royal Hallamshire Hospital. Between April-December 2023, funding from SQUIRE supported the piloting of a 'one stop multi-disciplinary clinic' located in the clinical rooms at the AWRC.

The clinic incorporated individualised consultations with an expert team who included a dietician, psychologist, nurse consultant, physiotherapist, research nurses and the Stroke Association. Patients attending the clinic completed a Patient Reported Experience Measure (PREM) but no further evaluation of this novel clinic model has been conducted. Funding from Sheffield Hospitals Charity has been secured to conduct a service evaluation.

The objectives of the service evaluation are:1. report clinic attendance rates and patient demographics2. report proportion of patients referred to secondary prevention services and/or Stroke Association3. analyse PREM data4. explore staff experience and perspectives of service delivery Objectives 1-3 will be achieved through descriptive analysis of medical records conducted by Nik Kudiersky who has an honourary contract with STH NHS Trust. Objective 4 will be attained through semi-structured interviews on Microsoft Teams with members of the clinical team. Data collection will be approved by the STH Clinical Effectiveness Unit
